These Terms of Service ("Terms") govern your access to and use of the website www.injuryaccidentnetwork.com (the "Site") and any related services provided by Injury Accident Network ("we," "our," or "us"). By accessing or using the Site, or by submitting a request through our intake form, you agree to be bound by these Terms. If you do not agree, please do not use the Site.
1. No Medical or Legal Advice
Injury Accident Network helps connect individuals affected by auto accidents and personal injuries with independent medical providers. We are not a law firm and do not provide legal advice or legal representation. Content on this Site is for general informational purposes only and is not medical advice. Your use of the Site or submission of an intake form does not create a doctor–patient relationship, an attorney–client relationship, or any professional engagement. Always consult a qualified medical professional or licensed attorney regarding your individual situation.
2. Use of the Site
You agree to use the Site only for lawful purposes and in a manner consistent with these Terms. You agree not to:
- Use the Site in any way that violates applicable law or regulation;
- Submit false, inaccurate, or misleading information through our intake form;
- Attempt to gain unauthorized access to any portion of the Site, its servers, or related systems;
- Interfere with or disrupt the integrity or performance of the Site.
3. SMS / Text Message Communications
This section describes our SMS text messaging program for individuals who submit our intake form.
Program Description. Injury Accident Network sends SMS text messages to phone numbers provided through our intake form for the purpose of scheduling, confirming, and following up on medical evaluations, and for related customer-care communications. This is a customer-care program. We do not send marketing or promotional text messages.
Consent. By submitting our intake form, you authorize Injury Accident Network to contact you by phone and email about your request. You may contact us by calling (385) 540-0683 or emailing [email protected] if you prefer not to submit the form.
SMS Opt-In. SMS text messaging is optional and requires explicit consent. To receive SMS text messages from us, check the SMS opt-in checkbox on the intake form before submitting. You can still submit the form without checking the box — we will simply contact you by phone or email instead. If you opt in, you may reply STOP at any time to cancel SMS messages without affecting other communications.
Message Frequency. Message frequency varies based on your interaction with us.
Message and Data Rates. Message and data rates may apply. Check with your mobile carrier for details.
Opt-Out (STOP). You may opt out at any time by replying STOP to any message you receive from us. After you reply STOP, we will send a one-time confirmation and will not send you further text messages unless you opt in again.

Carrier Disclaimer. Carriers are not liable for delayed or undelivered messages. Supported carriers include AT&T, T-Mobile, Verizon Wireless, Sprint, Boost, U.S. Cellular, MetroPCS, and others.
Privacy. Mobile information collected through our SMS program is handled in accordance with our Privacy Policy. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.
4. Intellectual Property
All content on the Site — including text, graphics, logos, and images — is the property of Injury Accident Network or its licensors and is protected by applicable intellectual property laws. You may not reproduce, distribute, or create derivative works from any content on the Site without our prior written permission.
5. Disclaimers
The Site and all content are provided on an "as is" and "as available" basis without warranties of any kind, whether express or implied. We do not warrant that the Site will be uninterrupted, error-free, or free of harmful components. Your use of the Site is at your own risk.
6. Limitation of Liability
To the fullest extent permitted by law, Injury Accident Network and its officers, employees, and agents will not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of the Site, the intake form, or our SMS program.
7. Changes to These Terms
We may update these Terms from time to time. Changes are effective when posted to this page with an updated effective date. Your continued use of the Site after changes are posted constitutes your acceptance of the revised Terms.
8. Governing Law
These Terms are governed by the laws of the State of Utah, without regard to its conflict-of-laws principles. Any dispute arising out of or relating to these Terms or your use of the Site will be brought exclusively in the state or federal courts located in Utah.
